Abstract
The purpose of this paper is to exhibit the significant increase in revenue of a single cosmetic surgery practice with the introduction of a novel artificial intelligence patient interactive system to increase patient throughput. This is a review of patient demographics and financial parameters with the introduction of the AI system over the course of 12 months. Investment totaled $20,364 over 12 months with generation of 2527 interactions, of which 107 patients booked consults, resulting in a total of 39 booked surgeries. Revenue generated over 12 months was $545,275, with resulting return on investment (ROI) of 2,578%. The conclusion of this paper is that addition of an AI patient interactive system could lead to significant ROI for cosmetic outpatient surgery practices.
Introduction

Materials and Methods
A single outpatient cosmetic surgery practice invested $1,697 per month into a novel AI patient interactive system from December 2024 through December 31, 2025. The services rendered by the system included a patient-interactive consultation interface on the cosmetic surgery practice website. Information provided to the patient is from a customizable Retrieval Augmented Generation (RAG) program provided by the AI company that allowed invested surgeons to train the system with data pertaining to their individual practice. The consultation interface included education on surgical options, post-operative outcomes, and before and after photos on request by the patient. The interface also incorporated patient demographic information into a Customer Relationship Management (CRM) system that was utilized for patient retention after leaving the site. A portion of patients were ultimately seen in pre-operative consultation, of which a number consented to and underwent surgery. Data collection was performed for 12 months after initiation of the AI-patient interface.
Results
A total of 2527 interactions occurred between prospective patients and the AI interface on the practice website from January 1, 2025 through December 31, 2025. This resulted in booking of 107 consultations. A total of 39 surgeries were booked and services paid in full from the consultation group. Cost of the AI patient consultation program was $1,697 per month, totaling $20,364 over 12 months. Total revenue generated on completed and pending services was $545,275, resulting in overall ROI of 2,578 % (Figure 1).
